In-house or outsourced

In-House Billing vs. Outsourcing: What Changes

Practice manager reviewing a billing report in a calm clinic

Keeping billing in-house means hiring and training billers, paying for software and clearinghouse fees, and covering the desk when someone is out. Outsourcing moves that work to a team that bills every day.

How it works

How Our Billing Process Works

Five connected steps, run by one team. Each one sets up the next so claims move and cash comes in.

Checking patient insurance coverage before a visit

1

Confirm coverage up front

Eligibility and benefits are verified before each visit, so every claim starts clean.

Clean claim ready for electronic submission

2

File claims that pass the first time

Charges are checked against payer rules, then submitted electronically or manually.

Insurance payments posted to patient accounts

3

Post every payment accurately

ERAs and checks are posted and matched to the original charge.

Denied claim traced to its root cause

4

Work denials before they age

Denials get first priority, and corrected claims go back out within filing limits.

Weekly billing report for a medical practice

5

Report back, week after week

Weekly or monthly reports show what was filed, what was paid and what's still open.

Why it matters

Why So Many Claims Get Denied

Denials are common and expensive. In a Premier Inc. survey, nearly 15% of claims sent to private payers were denied at first. More than half of those, 54.3%, were later overturned and paid, and fighting each one cost an average of $43.84.

On HealthCare.gov plans, insurers denied 19% of in-network claims in 2024, and fewer than 1% of those denials were appealed, according to KFF. Money left on unworked denials is simply lost.

  • Coverage problems. Inactive plans, wrong member IDs and missing authorizations. We verify eligibility before the visit.
  • Coding and modifier errors. Codes that do not match or are bundled away. We review charges before billing.
  • Missed deadlines. Claims filed or appealed too late. We track filing limits on every claim.
